1.中国大陆因私护照原件：

（1）行程结束后还有3个月以上有效期

（2）护照尾页本人签字（不能用铅笔）

（3）至少2页空白签证页

（4）如护照换发请一并提供老护照原件

4. 户口本

（1）整本复印件（原件请客人面试/递交材料时自行携带）；

（2）如是集体户口，可以提供集体户口页或户籍证明原件。

5.公司营业执照副本加盖公章或者组织机构代码证盖公章

6.在职证明：

（1）在职人员：领导签字（出行人不可以自己签名，且同行人不可以相互签名）、注明日期，内容需包含：公司的地址、电话、传真号码、签字人的姓名和职务、申请人的姓名、职务、收入、工作年限、准假证明（我司可提供模板）

（2）在校学生：准假证明（需用抬头纸打印并加盖公章和学校领导签名。内容必须写明是旅游目的）（我司可提供模板）

（3）退休人员：提供退休证复印件（原件请客人面试/递交材料时自行携带）

（4）无业人员：已婚者提供配偶工作单位出具的在职证明原件（英文版或中英文版），需使用单位抬头纸打印，加盖（公章）或（人力资源章）或（财务章）、签字并明确日期及包含如下信息：单位地址、电话、传真、单位签字人的姓名和职务、申请人配偶的姓名、职务、收入和工作年限

（5）单身／离异／丧偶：其他固定收入证明

7.资金证明

（1）银行存款证明原件（5万以上）存到回国后一个月。

（2）6个月以上的银行进出帐的记录或者6个月以上的存折进出记录盖银行红章，余额要求在5千元以上（尽可能多的体现余额，对出签有利）。

（3）房产证复印件，车辆行驶证复印件如有（原件请客人面试/递交材料时自行携带）

8.旅行证明

（1）具体行程计划（英文）

（2）全程机票预订记录（必须与您的英文行程单保持一致）（英文）

（3）全程酒店预订记录（必须与您的英文行程单保持一致，涵盖在申根地区逗留的全部时间）（英文）

9.医疗保险原件：保险期限要求覆盖整个行程，险额必须不少于30,000欧元。可以自行购买，也可我公司代买

10.同行关系证明（18周岁以下的未成年人提供）同行关系证明原件（请根据出行情况提供以下材料）：

（1）当未成年人单独出行时：由家长双方或法定监护人出具的出行同意书的公证书，并由外交部认证；当未成年人跟随单方家长旅行时：由不出行的另一方家长或监护人出具的出行同意书的公证书，并由外交部认证；

（2）亲属关系公证书，并由外交部认证。

（3）亲属关系公证书申请人必须为儿童本人，具体格式内容以当地公证处为主

11.结婚证已婚人员提供结婚证复印件；离异人员提供离婚证复印件。（原件请客人面试/

递交材料时自行携带）
