2.We have made available all information and records for the purpose of your audit. They

are free of material misstatements, or omissions.

我们已提供所有需要的数据及记录以供贵会/司审核使用。我们所提供的数据及记录均没有重大错误或遗漏。

3.We confirm that we do fully understand and accept the Terms and Conditions of the Care

Program listed on the registration form.
