2019年《EURETINA视网膜静脉阻塞诊疗指南》解读（全文） 2019年《EURETINA视网膜静脉阻塞诊疗指南》(简称指南)强调在诊断视网膜静脉阻塞(retinal vein occlusion，RVO)时应考虑鉴别诊断、是否存在黄斑水肿以及视网膜缺血程度[1,2]。该指南强调诊断RVO时还须结合患者临床表现和可能存在的循环系统异常进行鉴别，同时须判断患者是否存在RVO引起的黄斑水肿。目前，光相干断层扫描(optical coherence tomography，OCT)是明确患者是否存在黄斑水肿的有效方法。既往评估中央视网膜静脉阻塞(central retinal vein occlusion，CRVO)缺血程度的金标准是荧光素眼底血管造影(fundus fluorescein angiography，FFA)，但由于CRVO起病时往往有大量视网膜出血，对于评估造成一定困难，因此也可通过患者的临床表现、功能评价来评估其缺血程度。近年来，光相干断层血管成像(optical coherence tomography angiography，OCTA)和超广角荧光素眼底血管造影(ultra-widefield fluorescein angiography，UWF-FA)逐渐用于眼底疾病诊治的临床实践中，可无创评估眼底缺血的程度，而UWF-FA则可以观察到更为广泛的视网膜情况，这2项检查在RVO病情评估中各有优势，缺血指数可能也是有用的指标。指南推荐一旦确诊RVO，应进行全面查体和详细的病史询问，注意鉴别诊断、黄斑水肿是否存在、缺血程度，如果视网膜出血太多则可在治疗后且积血吸收再通过FFA进行评估。对于缺血和新生血管并发症的随访应该尤其注意患者视力、虹膜和眼底情况的随访。
