Flores Colque celebrated her 118th birthday last month. She is the oldest woman in Bolivia (玻利维亚). Following the death of a woman in Japan earlier this year, Flores Colque may now be the oldest person in the world. During her long life, Flores Colque has seen two world wars. Her home country of Bolivia had a major revolution. And the town where she is from has grown from 3,000 people to 175,000. Playing video games can be a fun way to unwind or spend time with friends. In schools, teachers use games like Minecraft to encourage teamwork and critical thinking.

(77)But for some players, gaming has become an unhealthy habit. Late at night, they are glued to a screen. School work suffers. The video-game world seems a friendlier place than the real one.

Mental health experts have taken notice. In June, the World Health Organization (WHO) added gaming disorder to its list of diseases and health conditions. A person may have the disorder if gaming has damaged his or her relationships with family and friends, and if it has affected his or her daily activities.

Not all experts agree that excessive gaming should be called a disorder. They say people hooked on video games may be suffering from other mental health problems that should be treated first. Others think WHO made the right call. Psychiatrist (精神科医生) Clifford Sussman treats gaming addiction(上瘾). (78) He says kids often have feelings of anxiety and loneliness. They get angry when a parent asks them to stop playing video games.

What makes video games addictive? Playing excites the brain′ s reward center. After a while, the brain becomes numb(麻木的to pleasure. You feel bored without a controller in your hand. So you play even more.

One key to healthy gaming is to track how long you play. Sussman recommends taking at least an hour-long break after every hour of play. That gives the brain time to recover.

Young gamers who think they might have a problem should seek help from an adult. Sussman suggests you first answer a single question; "Are you in control, or is the game in control?"

胃肠道间质瘤的治疗 发表时间:2010-08-25T16:09:32.513Z 来源:《中外健康文摘》2010年第16期供稿作者:刘文彬[导读] GIST不包括平滑肌瘤、平滑肌肉瘤、施万瘤和神经纤维瘤等,也不包括恶性黑色素瘤、纤维瘤病、血管肉瘤和Ewing肉瘤 刘文彬(黑龙江省中医药大学 150040) 【中图分类号】R730.5 【文献标识码】A 【文章编号】1672-5085 (2010)16-0107-01 【摘要】目的探讨胃肠道间质瘤诊断和治疗。方法回顾性分析2006年6月~2008年12月浙一医院外科收治并经病理证实的23例胃肠道间质瘤的临床资料。结果本组患者共23例,平均年龄54.5岁,男16例,女7例,均发生于胃部。病理诊断示:交界性4例,良性2例,恶性4例。其中6例行免疫组化检查。CD117,CD34(+)5例,SMA,S-100(-)6例。10例均行胃部分切除或大部切除术,其中1例行腹腔镜胃部分切除术。结论胃肠道间质瘤的诊断需要内窥镜、病理诊断和免疫组化的结合。手术切除是最主要治疗方法。【关键词】胃间质瘤胃肠道间质瘤治疗 胃肠道间质瘤(gastrointestinal stromal tumor,GIST)是一种免疫表型上表达KIT蛋白(CDll7)、遗传学上存在频发性c-kit基因突变、组织学上主要以梭形细胞和上皮样细胞呈束状交叉或弥漫性排列为特征的胃肠道间叶组织来源的肿瘤。 GIST不同于胃肠道的其他间叶源性肿瘤(gastrointestinal mesenchymal tumor,GIMT)。 GIMT除包括GIST外,还包括平滑肌瘤和肉瘤、施万瘤、神经纤维瘤、颗粒细胞瘤、脂肪瘤、球瘤、Kaposi肉瘤和血管肉瘤等。GIST不包括平滑肌瘤、平滑肌肉瘤、施万瘤和神经纤维瘤等,也不包括恶性黑色素瘤、纤维瘤病、血管肉瘤和Ewing肉瘤等。 一临床资料 一般资料:本组23例中男12例,女11例,年龄35~74岁,平均57.2岁;50岁以上16例(69.5%),40岁以下仅2例,所以病例诊断均经手术及病理结果证实,23例患者中胃间质瘤13例(56.5%),小肠间质瘤5例(21.7%),结肠间质瘤3例(13.0%),食管间质瘤2例(8.69%)。 二临床特点 1.性别和年龄肿瘤略多见于男性或男女相等,发病高峰年龄55~65岁,40岁以下少见。 2.部位整个消化道均可发生GIST,最常见于胃(60%~70%),以下依次为小肠(20%~30%),结、直肠和食管分别为18.1%和1.4%。 偶尔,相似于GIST的肿瘤也可发生于腹腔软组织(网膜、肠系膜)和腹膜后,称为胃肠道外间质瘤(extra-gastrointestinal stromal tumor,EGIST)。EGIST少见,约占GIST的3%~4%,肿瘤位于网膜和肠系膜者多于腹膜后者。 3.症状和体征肿瘤小,通常无症状。常在体检、X线、胃镜检查、CT检查或在其他手术时偶尔发现。肿瘤大,可出现症状,通常为非特异性,与部位有关。位于食管者可有吞咽困难;位于胃者可有不适、上消化道溃疡和出血;位于肠道者可有腹痛、腹块、梗阻、便血或穿孔等。恶性肿瘤可有体重减轻、发热,偶尔出现腹腔播散和肝转移症状。 三诊断和鉴别诊断 在做GIST诊断时,形态学标准仍具有不可替代的作用,Rosai认为,至少目前形态学比 CDl 17免疫染色在预测治疗反应和预后方面要好得多,并指出,用单个分子改变作为肿瘤分类惟一标准的危险性。 大多数GIST都有明确的临床病理特点,病理学家对诊断GIST是否必须用免疫组化证实KIT阳性的意见不完全一致。一些学者认为,不必做免疫组化可以诊断,而且KIT阳性肿瘤不一定都是GIST,文献报告,精原细胞瘤、肥大细胞瘤、恶性黑色素瘤均表达KIT,某些透明细胞肉瘤、Ewing肉瘤、原始神经外胚层瘤、Kaposi肉瘤、血管肉瘤、孤立性纤维瘤、隆突性皮肤纤维肉瘤、肌纤维母细胞瘤、纤维瘤病、分化良好脂肪肉瘤和转移性小细胞癌都可不同程度表达KIT。尽管如此,目前多数意见认为,为了使GIST诊断标准化和确定病人是否适宜应用伊马替尼治疗,应做KIT免疫染色。 四治疗 治疗上首选手术切除,对于术后切缘阳性的肿瘤或高度恶性者,应给予相应的辅助治疗;而对复发的或者转移者,甲磺酸伊马替尼是首选,根据具体情况采用并结合手术治疗。 完全切除是外科治疗的关键,此疾病具有很高的转移率和手术后复发率。手术的规范、合理是影响疗效的重要方面。切缘阴性的完整切除为外科治疗标准。不同于胃癌或肠癌,间质瘤的生长方式是膨胀式生长。切缘不需要达到5cm,一般认为,切缘2~3cm已经足够。其转移方式与上皮来源的癌不同,以腹腔种植和血行转移为主,淋巴结转移的发生率低于10%,因此不主张进行淋巴结清扫。 无瘤操作是降低手术后复发的重要环节,假包膜破裂不但可能导致出血,而且显著增加肿瘤种植转移的危险。最佳手术方式是包括肿瘤组织和周围部分正常组织在内的完整切除。扩大切除并不能提高生存率,只有当肿瘤侵犯邻近器官时,为防止种植播散和切缘阳性,可将邻近脏器一并切除。 手术前穿刺可能造成肿瘤破裂和出血,增加肿瘤播散风险。因此,对于临床怀疑为 GIST者,手术前不做穿刺活检。参考资料 [1]朱雄增,侯英雄.对胃肠道间质瘤的再认识[J].中华病理学杂志,2004,33(l):3.
